Indications for Hospice
The Hospice philosophy is about providing comfort and focusing on quality of life. Patients must have a terminal illness and a prognosis of six months or less if the disease runs its natural course, and be certified by a Physician.

A patient must have an illness that is terminal (a prognosis of 6 months) and the patient and/or family has elected palliative care.
Palliative Performance Scale (PPS) rating of 50%-60%, or, Dependence in 3 of 6 Activities of Daily Living (ADLs).
Three hospitalizations or ED visits, Decrease in tolerance to physical activity, or Decrease in cognitive ability.
We tailor our care to address the specific needs of your patients in nursing homes, assisted living communities, residential care facilities, general inpatient beds, and inpatient hospice units.
